It is that time of the year again to get your OSHA 300A posted (technically as of February 1st, 2026). Starting in 2024 Cardinal has been tracking recordable injuries and illnesses in PrismHR, our new payroll platform.
Some advantages of the new system:
- Our Risk Team enters in injury data quickly and keeps it up to date. For example, how many days of lost time.
- OSHA 300 logs are updated daily in the new system (count of days/lost time/light duty and has the ability to produce a report as of today’s date).
- Employers have access to this report 24/7, even when Cardinal is closed and Oregon OSHA is standing at your doorstep*.
*If Oregon OSHA is at your doorstep and you are having trouble with the report, please call Cardinal for help. Cardinal staff have access to this report and can get you a copy quickly.
How to print and post your OSHA 300a and get a copy of your 300 for your records:
To access the OSHA 300 and 300A:
- Log into PrismHR.
- Go to Reports -> choose OSHA report -> select Year 2025, click Run.
- In the new pop-up window, look for the download icon at the top right of the screen and click it to download the report.
OSHA 300 Report
The OSHA 300 report is blank unless you have an incident that is recordable.
A common misconception is that all workers’ compensation claims are recordable. Cardinal’s Risk Team makes that determination but reach out if you have questions. The OSHA 300 is an ongoing report with details of a recordable accident. This report is not to be posted, just place it in your records.
OSHA 300A Report
The OSHA 300A is the annual report, summarized and posted from February 1st to April 30th, 2026. Please remember that the highest-ranking supervisor/manager at that specific workplace location must sign the 300A.
